The Practicum Assessment Form is to be completed at mid-point and at the end of each practicum placement.
- Each assessment must be signed by the appropriate parties.
- A copy of the assessment should be kept by the Associate Teacher.

The Practicum Review Form is to be completed by the Associate Teacher in consultation with the Faculty Liaison for a Candidate in difficulty.
