Concurrent Placement Information Form

For candidates, Host Teachers and Principals

Concurrent Placement Arrangement Form

For candidates enrolled in PRAC 111, PRAC 210 (formerly PRAC 116), PRAC 211 (fomerly PRAC 131), PRAC 310 (formerly PRAC 121), and PRAC 311 (formerly PRAC 136).

The on-line Placement Arrangement form will be posted by September.

 

Work/Education Placement Agreement form

For liability and insurance purposes, Queen's University Environmental Health and Safety department requires all Teacher Candidates to complete and submit a Work/Education Placement Agreement form.  This form must be signed by all parties and submitted to the Practicum Office, (Queen's) or Concurrent Education Office (Queen's-Trent) with a copy kept on file at the Host School.
