Practicum Assessment Form (PDF interactive version)
Complete during and after a practicum placement.
- All copies of each assessment must be signed by the appropriate parties.
- The Teacher Candidate will submit the signed copies of all summative assessments to the Practicum Office, Faculty of Education.
- Signed copies of the summative, formative and self-assessments must be submitted to the Faculty Liaison.
- Teacher Candidates keep the original copy of all assessments.
